How to Choose DMG

Choosing your Support Coordination Agency is your decision. On the NJ DHS DDD website Care Management/Support Coordination | Division of Developmental Disabilities review section carefully. To choose DMG, select Decennium Management Group on your Support Coordination Agency Selection Form sca-change-form(fillable).docx and submit it to DDD following the instructions on the form. Once you do, here is what happens:

• Your coordinator gets to know you, your family, and your goals.

• We research the federal, state, and local services and programs that fit your needs.

• We build your Individualized Service Plan to guide your supports.

• Once you are happy with it, your coordinator helps put the plan into action.

• We stay in touch regularly to make sure your plan still fits your life.
